Best AI Tools for Dentists in 2026: A Vendor-by-Vendor Shortlist Sorted by Clinical Job, PMS Compatibility, and BAA Terms

Best AI Tools for Dentists in 2026: A Vendor-by-Vendor Shortlist Sorted by Clinical Job, PMS Compatibility, and BAA Terms

How many of the AI vendors in your inbox this quarter can write a finding, a booked appointment, or an eligibility result back into Dentrix, Eaglesoft, or Open Dental without someone retyping it? If you run operations for a group practice or a DSO, you already know that question removes more vendors from a shortlist than any demo does.

Our dental AI software category map sorts the market by type of tool. This post goes one level down and names vendors, organized by the five clinical and front-office jobs where dental AI is in production in 2026.

Which AI tools should a dental group evaluate first? Start with FDA-cleared radiograph detection, then insurance verification and phone agents. In every category, PMS write-back and a signed BAA decide the shortlist.

How We Scored Each Vendor

Every vendor below is checked against the same four criteria, so you can compare across categories as well as within one. These criteria map to the questions your compliance lead, your IT lead, and your CFO will each ask before signing.

The criteria include:

  • FDA status. Whether the product is a medical device and, if so, which indications its 510(k) clearance covers. Clearance is granted per indication, so a vendor cleared for caries detection is not automatically cleared for periapical radiolucency or bone-level measurement.
  • PMS read and write. Which practice management systems the tool can read from, and which it can write back to. Read-only integrations still leave a person retyping data, which is where errors and labor costs accumulate.
  • BAA availability. Whether the vendor signs a business associate agreement, and whether that BAA covers its subprocessors, including any large language model provider sitting behind the product.
  • Pricing model. Per location, per provider, per call, per check, or flat subscription. Most dental AI pricing is quote-based, so the structure matters more than the list number when you model cost across 10 or 100 offices.

Keep in mind that integration lists and clearance scopes change quarterly. Treat the tables below as a starting shortlist, then confirm each cell with the vendor and against the FDA 510(k) database before contracting.

Radiograph Detection: The Only Category Where FDA Clearance Applies

Radiograph analysis is the most mature dental AI category and the only one on this list regulated as a medical device. Software that flags caries, calculus, bone loss, or periapical findings on bitewings and periapicals is computer-aided detection, and it needs 510(k) clearance for each claim it makes.

Do dental radiograph AI tools need FDA clearance? Yes. Software that flags caries or bone loss on radiographs is a device and needs 510(k) clearance per indication. Phone, scheduling, and verification tools do not.

The established vendors here are Overjet, Pearl, and VideaHealth, with Denti.AI as a smaller entrant that also offers voice charting. Each has cleared indications on 2D radiographs, and each sells primarily through imaging and PMS bridges rather than a standalone viewer.

OverjetCaries detection, bone-level measurement, claims review510(k)-cleared for caries detection and bone-level measurement on 2D radiographsReads imaging and charts from Dentrix, Eaglesoft, Open Dental, Denticon; write-back depth varies by PMSYesPer-location subscription, quote-based
Pearl (Second Opinion)Multi-condition detection on bitewings and periapicals510(k)-cleared; confirm each indication you plan to useReads via imaging bridges across major PMS platforms; findings surface in its own overlayYesPer-location subscription, quote-based
VideaHealthCaries and multi-condition detection, DSO analytics510(k)-cleared (Videa Caries Assist and later indications)Enterprise integrations with major PMS and imaging platforms; DSO-orientedYesEnterprise contract, quote-based
Denti.AIDetection plus voice chartingHas announced 510(k) clearance for its detection module; verify scopeIntegrations with common PMS platforms; confirm write-back per fieldYesPer-provider or per-location, quote-based

The differentiator between these vendors is rarely detection accuracy alone, since all of them publish reader-study results from their clearance submissions. What separates them in production is where findings land: inside the PMS chart, inside a separate viewer, or inside a claims workflow. For the evaluation method behind these comparisons, see our guides to AI radiograph analysis and AI caries detection.

Remember that a cleared model can still drift when your imaging hardware changes. A sensor swap across 20 offices changes the input distribution, which is why model drift monitoring belongs in the contract conversation, not after go-live.

Charting And Clinical Notes: Voice In, Structured Data Out

Charting AI falls outside FDA device rules because it records what the clinician says rather than making a diagnostic claim. That said, the stakes are still high, since a mis-transcribed tooth number or pocket depth becomes a permanent part of the legal record.

What should dental charting AI write back? Tooth number, surface, and pocket depth as structured fields in the PMS chart. A transcript pasted into a free-text note still leaves someone re-entering data.

Bola AI focuses on voice-driven periodontal charting, letting a hygienist call out six-point readings without a second person at the keyboard. Denti.AI Voice covers perio and restorative charting with a similar hands-free model, and some radiograph vendors are extending into note drafting from imaging findings.

VendorClinical jobFDA statusPMS read / writeBAAPricing model
Bola AIVoice periodontal and restorative chartingNot a medical device; clearance not applicableWrites perio charts into supported PMS platforms including Dentrix, Eaglesoft, and Open Dental; confirm version supportYesPer-provider or per-operatory subscription
Denti.AI VoiceVoice perio and restorative charting, note draftingNot a medical device for chartingIntegrations with common PMS platforms; confirm field-level write-backYesPer-provider subscription

When you pilot a charting tool, test it against your own clinicians' accents, operatory noise, and shorthand rather than the vendor's demo audio. Our AI dental charting guide covers the tooth-numbering and surface-coding edge cases that break weaker systems, including FDI versus Universal numbering in multi-region groups.

AI Phone Agents: The Category Where Write-Back Matters Most

Phone agents answer inbound calls, handle after-hours overflow, confirm appointments, and in the stronger implementations book directly into the schedule. Because these systems are typically built on a general-purpose LLM plus speech models, the BAA chain matters more here than anywhere else on the list.

Can an AI phone agent book appointments directly? Only with write access to the PMS schedule. Without it, the agent creates callback tasks, which moves front-desk work instead of removing it.

Arini builds AI receptionists specifically for dental practices, with direct PMS scheduling integration as a core feature. Weave, which many practices already use for phones and messaging, has added AI receptionist capabilities on top of its existing PMS sync.

Before signing, place 20 scripted test calls covering a new patient, a reschedule, an emergency, a Spanish-speaking caller, and a caller who tries to extract another patient's information. The last scenario is a real attack surface, and our post on prompt injection in clinical AI explains why voice agents are exposed to it. For operational benchmarks, see our deeper review of AI phone agents for dental practices.

Insurance Verification: Where The Math Is Easiest To Prove

Insurance verification is the front-office job with the clearest return, because the labor is measurable and repetitive. Consider a six-operatory office running 40 verifications a day at roughly 10 to 15 minutes each, which works out to 7 to 10 staff hours daily before a single patient is seated.

How does AI insurance verification work? It pulls eligibility and benefit data through payer APIs, EDI 270/271 transactions, or portal automation, then writes coverage, frequencies, and remaining maximums into the PMS.

The vendors here split into two groups. Some, like Onederful and DentalXChange, provide eligibility data and connectivity that other tools build on, while others, like Zuub and Vyne Dental, deliver a finished verification workflow to the front desk.

ZuubAutomated eligibility and benefits breakdownsNot a medical deviceIntegrates with major PMS platforms; writes benefit summaries backYesSubscription, quote-based

DentalXChangeEDI clearinghouse, eligibility, claimsNot a medical deviceConnects through PMS clearinghouse settingsYesPer-transaction or monthly plan

Note that the hard part is not the 271 response but the breakdown: frequencies, waiting periods, downgrades, and history that payers often return incompletely. Our AI insurance verification guide covers how to measure accuracy against a manual baseline before you cut staff hours.

Scheduling Optimization: Filling The Chair You Already Have

Scheduling AI ranges from online booking with PMS sync to optimization engines that recommend which unscheduled treatment to pull into an open slot. Because this category reads production data across the whole schedule, it often doubles as the analytics layer for a group.

NexHealth provides online booking, forms, and messaging with a documented integration layer across major PMS platforms. Dental Intelligence focuses on schedule analytics and morning-huddle reporting, and Weave appears again here through its scheduling and reminder features.

NexHealthOnline booking, reminders, patient formsNot a medical deviceReads and writes appointments across Dentrix, Eaglesoft, Open Dental and othersYesPer-location subscription
Dental IntelligenceSchedule analytics, huddle reports, patient follow-upNot a medical deviceReads PMS data broadly; write-back varies by featureYesPer-location subscription
WeaveReminders, confirmations, scheduling requestsNot a medical deviceExisting PMS syncYesPlatform tiers

The highest-value use of scheduling AI is usually not new-patient booking but recovering diagnosed work that never made it onto the calendar. Our posts on AI scheduling optimization and unscheduled treatment recovery show how to size that opportunity before you buy anything.

BAA Terms: What To Read Before You Sign

Every vendor on this shortlist will sign a BAA, so the existence of one does not distinguish them. What distinguishes them is what the BAA covers, and that requires reading the subprocessor list, not just the signature page.

What should a dental AI BAA cover? Every subprocessor touching PHI, including the LLM and speech providers, plus retention limits, a no-training clause for PHI, and breach-notice timelines shorter than HIPAA's 60-day outer limit.

Here is what to confirm in each agreement:

  • Subprocessor coverage. If the phone agent runs on Claude through Bedrock or on GPT-5 through Azure OpenAI, the downstream BAA must exist and must cover that exact service. A vendor calling a consumer API endpoint has a gap no paper agreement fixes.
  • Training use. The contract should state that PHI, call recordings, and radiographs are not used to train shared models without explicit written consent.
  • Retention and deletion. Call audio and transcripts should have a defined retention window and a verifiable deletion path at contract end.
  • Breach notification. HIPAA allows up to 60 days, but your operations need far faster notice to meet your own obligations, as covered in our AI breach notification guide.

Pair the BAA review with the vendor's SOC 2 Type II report, using the checklist in our post on SOC 2 for clinical AI vendors. For the full compliance framework, see HIPAA requirements for clinical AI in dentistry.

How To Run This Shortlist In Your Organization

A shortlist only helps if it shortens the evaluation, so run it as a sequence rather than five parallel bake-offs. Here is the order we recommend for a multi-location group:

  1. Inventory your PMS versions first. A group running Dentrix G7 in some offices and Dentrix Ascend in others will get different integration answers from the same vendor.
  2. Gate on BAA and SOC 2 before demos. This removes vendors whose subprocessor chain will not survive your compliance review, before anyone gets attached to a product.
  3. Test write-back in a sandbox. Ask each finalist to write a real record into a test database, then confirm it lands in the correct field and that a retry does not create a duplicate, the failure mode covered in our post on PMS write-back idempotency.
  4. Run shadow mode for 30 days. Let the tool operate alongside your current process without acting on its output, and measure agreement against your staff.
  5. Set an eval cadence before go-live. Define the accuracy thresholds that would trigger a rollback, using the framework in our clinical AI evals guide.

All of this adds up to a buying process that rewards the vendors who integrate well rather than the ones who demo well. After all, the tool that writes clean data into your PMS will outperform a more impressive model that leaves your front desk retyping its output.
